What is sexual dysfunction?

Sexual Dysfunction is any physical or psychological problem that prevents you or your partner from getting sexual satisfaction. Male sexual dysfunction is a common health problem affecting men of all ages, but is more common with increasing age. Treatment can often help men suffering from sexual dysfunction.

The main types of male sexual dysfunction experienced by men are:

  • ED - Erectile Dysfunction, difficulty getting or maintaining an erection.
  • PE - Premature ejaculation characterized by reaching orgasm too quickly
  • Delayed or inhibited ejaculation reaching orgasm too slowly or not at all
  • Low libido, loss of sex drive with a reduced interest in sex or intimacy

What causes sexual dysfunction in men?

Physical causes of Male Sexual Dysfunction may be:

Psychological causes of ED and PE include:

  • Anxiety about sexual performance
  • Marital discord, financial or relationship problems
  • Depression, sadness, stress, feelings of guilt
  • Emotional effects of past sexual trauma or abuse
  • Work-related stress and anxiety

Male Sexual Dysfunction

How does sexual dysfunction affect men?

Male Sexual Dysfunction. The most common problems men face with sexual dysfunction are troubles with premature ejaculation, getting and keeping an erection, weak erections and reduced sexual desire. Ejaculatory disorders, loss of libido and erectile problems can also be caused by lower than normal testosterone levels. Testosterone in Men

Ejaculatory Disorders in Men

Problems with ejaculation can be mental, emotional or physical and can be encountered at any age.

  • PE - Premature ejaculation — ejaculation that occurs before or too soon after penetration
  • Delayed Ejaculation - Inhibited or delayed ejaculation — ejaculation does not happen or takes a very long time
  • Retrograde ejaculation — at orgasm, the ejaculate is forced back into the bladder rather than through the end of the penis

What causes PE? Although the exact cause of premature ejaculation (PE) is not known although many practitioners attribute both physical and pschological causes to ejaculatory problems in men. While in many cases premature ejaculation is caused performance anxiety during sex, other factors such as weak muscles and overly sensitivity can also cause PE. Other cause can include:

  • Stress - Physical or Emotional
  • Depression, Mood Disorders
  • History of Sexual Abuse or Repression
  • Low Self- esteem, Lack of Self-Confidence
  • Long Period of Celibacy before Sexual Activity
  • Lack of Communication, Trouble with Intimacy or Conflict with a sexual partner

Erectile Dysfunction Studies demonstrate that the serotonin, a natural chemical that affects mood may play a role in ejaculatory problems. Certain prescription drugs including antidepressants and alcohol can affect men's ejaculation, as can penile nerve damage, disc or spinal cord injury.

Physical causes for inhibited or delayed ejaculation may include chronic health problems such as coronary or cardiovascular disease, obesity, prescription medication side effects, alcohol abuse, injuries or surgeries. The problem can also be caused by psychological factors such as depression, sexual performance anxiety, stress, or relationship problems.

Retrograde ejaculation where sperm does not exit the body is most common in males with diabetes who suffer from diabetic nerve damage. Problems with the nerves in the bladder and the bladder neck force the ejaculate to flow backward. In other men, retrograde ejaculation may be a side effect of some medications, or caused by a surgical procedure on the bladder neck, testicles or prostate. In either case infertility can result.

Erectile dysfunction (ED) and What to do about it.

Erectile dysfunction (ED) or male impotence is the inability to get and keep an erection for sexual intercourse. ED is quite common, with studies showing that about one half of American men over age 40 are affected and show some form of impotence even if intermittently. Causes of ED include:

  • Diseases affecting blood flow such as hardening of the arteries
  • Nerve disorders including penile insensitivity or curvature
  • Stress, relationship conflicts, depression, and performance anxiety
  • Injury to the penis or to the testicles including the leydig cells
  • Chronic illness such as diabetes and high blood pressure
  • Prescription drugs and even some over the counter supplements
  • Unhealthy habits like smoking, drinking too much alcohol, overeating, and lack of exercise

Low Libido (reduced or loss of sexual desire)

Low libido means your desire or interest in sex has decreased or is non-existent. This male health condition is often linked with low levels of the male hormone testosterone, especially in men older than 40 or 50. Testosterone is the hormone of desire in both men and women and it promotes and maintains a healthy sex drive, sperm production and potency, lean muscle, proper fat distribution, thick hair, and strong bones. Low testosterone levels can negatively affect your body, sex drive, body composition and mood.

Reduced sexual desire may also be caused by depression, anxiety, or relationship difficulties. Diabetes, high blood pressure, and certain medications like antidepressants may also contribute to a low libido.

How is male sexual dysfunction diagnosed?

Your doctor may begin the diagnosis process with a blood test and physical exam. Physical tests may include:

  • Blood tests to check your testosterone levels, PSA levels, HGH levels, blood sugar (for diabetes), and cholesterol
  • Blood pressure check
  • Rectal exam to check your prostate
  • Examination of your penis and testicles

More ED Testing

Other tests can show if you have problems with the nerve impulses or blood flow to the penis.

Your sexual health doctor may also ask questions about your symptoms and your medical and sexual history. Though these questions may seem very personal, do not be embarrassed. It is important to answer honestly so the best treatment can be recommended. You may be sent to a different type of doctor (urologist, endocrinologist or sex therapist, for example) who can help you.

How is male sexual dysfunction treated?

Many cases of sexual dysfunction can be corrected by treating the mental or physical problems that cause it. Treatments include:

ED Medications that improve Blood Flow and Penile Function

  • Medications – drugs that help improve sexual function by increasing blood flow to the penis. Sildenafil (Viagra®), vardenafil (Levitra®), and tadalafil (Cialis®), are safe and effective for most men.
  • Injectable ED Medications – drugs that help alleviate impotence, including vasofilators and prostaglandons, Caverject, Edex, Muse, Trimix, Bimix, Quadmix, Priapus Shot (P-Shot®).
  • Trimix Injections a tri-mixture of 3 erectile prescription drugs - Alprostadil, Papaverine, and Phentolamine - Trimix Injections for ED
  • Hormone therapy – low levels of testosterone raised by hormone replacement therapies that include injections, patches, or gels.
  • Psychological therapy – a psychological counselor to help you address feelings of anxiety, depression, fear, or guilt that may affect sexual function.
  • Mechanical aids – vacuum devices, erectile medications, testosterone supplementation, and penile implants that can help some men with erectile dysfunction.

ED Treatment Alternatives

ED Surgery Vascular reconstructive surgery is a traditional way urologists try to improve blood flow to the penis to help men with erectile dysfunction get and maintain an erection. Oral male enhancement ED drug medications Cialis, Levitra, Staxyn, and Viagra are prescribed to Treat ED and new methods of ED treatment include the P-Shot® (Priapus Shot), and Low Intensity Shockwave Therapy or Acoustic Wave Therapy for ED using ultrasound to improve penile vascular health and blood flow.

Can male sexual dysfunction be prevented?

Efforts to avoid ED. While male sexual dysfunction cannot be prevented, dealing with the causes of the dysfunction can help you better understand and cope with the problem when it happens. To help maintain good sexual function:

  • Follow your doctor’s treatment plan for any of your medical/health conditions.
  • Limit your alcohol intake. Excess Alcohol consumption can be directly linked to erectile problems.
  • Quit smoking. Cigarettes and cigars rob your body of needed vitamins and restrict penile blood flow.
  • Get treatment if needed for any emotional or psychological problems such as stress, depression, and anxiety.
  • Communicate better and more often with your partner.

Can Anabolic Steroids help with ED or cause ED?

Anabolics and ED. Many men searching for a way to build big muscles abuse anabolic steroids. Anabolic Steroids are synthetic chemical substances that help boost natural testosterone production. The problem with steroids vs natural testosterone supplementation with bio-identical hormones is that steroids when used in mega doses can have a negative effect on the body. When taken in very high amounts the endogenous or natural testosterone level can fluctuate widely leading male sexual dysfunctions including erectile dysfunction especiallly when androgen levels go low. When the body thinks it has enough testosterone in the blood, natural secretion tends to shut down. Since a man's body produces androgen hormones very similar to anabolic steroids in the testes, steroid overuse tends to cause the male body to stop producing those androgen hormones which can lead to symptoms of ED, weak or soft erections. Equally, long term steroid use can cause a reduction in the levels of gonadotropins and high blood pressure which has been clinically shown to cause sexual function problems.

Low Dose Steroid use and ED. when taken in low doses steroids and steroid hormones such as testosterone have a positive therapeutic effect on sexual desire and functioning and is used in Male HRT to treat erectile dysfunction. As men reach middle age a reduction in the level of blood serum testosterone occurs and can cause erectile dysfunction symptoms including the frequency and firmness of erections. When men who are going through Andropause (hypogonadism) have lowered sex drive, weak or inability to have erections due to androgen deficiency, doctors may prescribe steroids along with prostaglandin and vasoldilators in a physician supervised treatment where those men usually regain sexual desire and function. Read more detail about Anabolic Steroids like Testosterone for Men.

What causes male infertility? Male infertility is due to low sperm production, abnormal sperm function or blockages that prevent the delivery of sperm. Symptoms of Male infertility can have many causes and urologists have a variety of tests to help make a diagnosis and propose solutions.

  • Lack of fertility.

What is Male Infertility?

There are many causes for infertility in both men and women and infertility is a bigger problem than most realize.
