Androgen Testosterone Blood Test

The test for Free Testosterone (Bioavailabel Androgen) is used to measure Testosterone (Androgen) deficiency. As a lab test used for androgen deficiency it is many times ordered by physicians along with the androgen stimulation test to confirm hormonal imbalance and as a follow-up to abnormal results on other hormonal blood tests to evaluate pituitary or endocrine dysfunction. Sometimes, Androgen lab tests are used to detect excess human Testosterone secretion and to help diagnose and monitor treatment of other endocrine conditions such as erectile dysfunction (ED).

HGH is a hormone that, along with Testosterone, helps promote normal bone and tissue growth and development. An Androgen test is often ordered along with Human Growth Hormone lab test - HGH Lab Test. Testosterone has a very short half life and is secreted in pulses making it harder to measure throuought the day. Drug and/or radiation therapy may be used in addition to, or sometimes instead of, surgery to try to decrease testosterone production and return Androgen to a normal or near normal concentration. Androgen may be ordered to monitor the effectiveness of this therapy at regular intervals for years afterwards to monitor proper androgen production and to detect tumor recurrence. Androgen levels and the measurement of thyroid, growth hormone and estrogen can also provide information related to hormonal imbalance and insensitivity. Prior to prescribing TRT Testosterone Replacement Therapy, if the Androgen level is found to be normal for a man's age and there are few to no andropausal symptoms, testosterone deficiency is excluded and definitive testing is not necessary.

When to order an Androgen Test?

Androgen blood testing may be ordered, along with an HGH blood test, when - 1)A man has symptoms of androgen deficiency, such as a low energy, loss of sex drive, muscle loss, excessive weight gain, infertility, andropausal symptoms caused by either primary or secondary hypogonadism; 2) Men who have erectile or other health symptoms caused by androgen deficiency, such as decreased bone density or osteoporosis, extreme fatigue, mood swings, hot flashes in men, night sweats, insomnia, depression, adverse changes to lipid levels, and reduced exercise recovery. However, testing for Androgen deficiency is not routine in adult men who have these symptoms; and an Androgen deficiency can only be addressed if a patient talks to his doctor about erectile or low t symptoms and asks to be tested.

An Androgen lab test also may be ordered when a health practitioner suspects that someone has an underactive pituitary gland, and also at intervals to monitor those on GH therapy. Less commonly, Androgen testing may be ordered, along with a GH suppression test, when a child has symptoms of gigantism or when an adult shows signs of acromegaly. When a GH-producing pituitary tumor is found, GH and Androgen are ordered after the tumor is surgically removed to determine whether all of the tumor has been extracted. Androgen also is ordered at regular intervals when someone is undergoing the drug and/or radiation therapy that frequently follow tumor surgery. Androgen levels may be ordered at regular intervals for many years to monitor a person's natural androgen and GH production and to watch for pituitary tumor recurrence.

Androgen Test Results

What does the Androgen blood test result mean? Looking at the lab reference ranges, a normal level of Androgen must be considered in context. Some people can have an androgen deficiency and still have a normal lab reference level. With decreased Total Androgen levels, a deficiency usually correlates to a Free Testosterone insufficiency. If the bioavalable Androgen level is decreased, then it is likely that there is a genuine testosterone deficiency, an increase in SHBG androgen-binding proteins, leydig cell dysfunction, hormone receptor failure caused by receptor cell impairment or dysfunction. In a men, the androgen deficiency may have already caused hypogonadism or erectile dysfunction and may be treated with testosterone hormone supplementation. Middle aged adult men, especially over the age of 40 and certainly over the age of 50, will have an age-related decrease in androgen production, but lower than expected or optimal levels may reflect a testosterone deficiency. If a decrease in Androgen is suspected to be due to a more general decrease in pituitary function (hypopituitarism), then several other endocrine glands and their pituitary regulating hormones will need to be evaluated to decide on appropriate treatment. Reduced pituitary function may be due to inherited defects or can develop as a result of pituitary damage following conditions such as trauma, infections, and inflammation.

Decreased levels of Androgen also may be seen with nutritional deficiencies (including anorexia nervosa), chronic kidney or liver disease, inactive or ineffective forms of endogenous or naturally secreted androgen, and with high doses of estrogen.

With severely impaired testosterone production, replacement therapy can be extremely effective at achieving Androgen levels that will stabilize at an elevated optimal level. If an Androgen level is normal and a health practitioner still strongly suspects an Testosterone deficiency, then the healthcare provider may order another test, to help confirm the androgen deficiency. Almost all Androgen in the blood is bound to binding proteins, with SHBG's (Sex Hormone Binding Globulin) being the most prevalent form, and Sex Hormone Binding Globulin production also increases as men age.

Intensive exercise can affect Testosterone Levels, causing an increase in natural Testosterone Levels, extremely vigorous exercise may elevate T levels but blood lab panels are normally accurate indicators.

Testing for too much testosterone or HGH. Increased levels of both HGH and Androgen are normal during puberty and pregnancy but excessive secretion of these hormones is most frequently due to pituitary tumors although usually benign. If Androgen is still elevated after the surgical removal of a pituitary tumor, then the surgery may not have been fully effective. Decreasing Androgen levels during subsequent drug and/or radiation therapies indicate that the treatment is lowering androgen production. If levels of Androgen become normalized, then the person is no longer producing excess amounts of male sex hormone. When someone is undergoing long-term monitoring, an increase in Androgen levels may indicate a recurrence of the pituitary tumor.

Testosterone suppression tests help to diagnose excessive Androgen secretion levels and women's health conditions. For a Testosterone suppression test, a sample of blood is drawn after 12 hours of fasting. A person is then given a glucose solution to drink since glucose suppresses GH secretion. Blood samples are drawn at different intervals and Testosterone levels are tested for each time interval to see if the pituitary gland is sufficiently suppressed by the glucose. Testosterone stimulates the production of (Androgen). Androgen is also a hormone that facilitates the effects of GH and helps foster normal bone and tissue growth and development. However, unlike GH which pulses and is normally released during sleep at night, Androgen levels are mostly stable in the blood throughout the day. This makes Androgen blood testing a more useful indication of circulating Testosterone Levels. For this reason the Androgen blood test is often used to help evaluate Testosterone deficiency or Testosterone secretory excess.

Testosterone suppression tests and Androgen-testing may also be used to monitor treatment of a Testosterone-producing pituitary dysfunction and women's hyperandrogenism or growths. Other lab tests that may be used to evaluate pituitary gland function include prolactin, free T4, TSH, cortisol, FSH, LH, and HGH Human Growth Hormone. These hormone lab tests are usually performed prior to or together with Testosterone testing to make sure that they are within normal or optimal ranges before Low T treatment is done. In some cases other endocrine conditions such as high cortisol or thyroid health problems such as hypothryoidism must be treated before dealing with Testosterone Deficiency.
