HGH and testosterone production increases rapidly at the onset of puberty and starts to decrease there after. It decreases rapidly after age 40 (to 20% to 80% of peak level by age 70). Almost all men and women in the United States experience adult hormone deficiency after the age of 50 and need to receive treatment for negative health symptoms.

How can I increase my HGH levels? There are few effective ways of increasing HGH: One is taking an injectable human growth hormone releasing hormone (GHRH) or Sermorelin Actetate, substances that increase the natural secretion of HGH by the pituitary gland. Hormone releasing agents have the most successful results in those patients between the ages of roughly 30 to 40 years. For most patients over 40, authentic injectable HGH is the most effective treatment option. HGH pills, liquids and oral sprays have almost no effect on hormone levels. The HGH molecule is too large to be absorbed through the mouth and it is destroyed in the digestive tract before ever being absorbed into the blood stream. Some Amino Acid anti-aging supplements do have a slight effect on HGH secretion becuase they conatin amino acids such as L-Gluatmine and L-Arginine. However, in adult men and women over the age of 40 increase in amino acids with supplements has almost no effect of natural HGH levels.

Signs and Symptoms
Men & Women are generally reluctant or unwilling to acknowledge that the syndrome has crept up on them.  Many family doctors are unaware of or not trained to diagnose GH deficiency. The symptoms are not as overwhelming as the dramatic changes women experience during menopause with steep declines in estrogen and progesterone. About 40% of men and women in their 40s, 50s, and 60s will experience some of the symptoms of hormonal imbalance, including insomnia, extreme fatigue, lethargy, depression, weight gain, hot flashes, night sweats, irritability, mood swings, female sexual and male erectile dysfunction.

HGH levels begin to decrease for a number of reasons. Declines in GHRH and GHRP causes the pituitary to decrease natural HGH secretion. Growth hormone releasing hormone and releasing peptide decline with age and some physicians may prescibe GHRH or GHRP to help stimulate the pituitary to increase its production of HGH. Somatostatin inhibits HGH secretion and Somatostatin increases with age further blocking the release of HGH by the pituitary gland. Since natural or endogenous production of HGH is controlled by these other hormones GHRH, GHRP, somatostatin, natural changes in their levels impede growth hormone secretion causing a deficiency. Three major analogs of growth hormone releasing hormone include Sermorelin, Tesamorelin and Ipamorelin.

Prostate issues during Andropause. As men pass 50, they tend to develop an enlarged prostate.  As the gland increases in size, it squeezes the urethra, often causing increased urinary frequency, a weaker flow and difficulty beginning urination.  The current treatments include finasteride (Proscar) and Saw Palmetto (made from the berries of a plant native to the American Southeast) which reduces the size of the prostate in only four to six weeks and is relatively effective.  Zinc is also used to maintain a healthy prostate. In addition to enlarged prostate, other prostate problems include prostatitis, and prostate cancer.  It has been stated that men have odds of 100% of experiencing one of these three disorders. HGH should not be taken if a man has prostate cancer.

Adipose Body Fat increases with age. HGH has been clinically shown to help reduce adipose or visceral body fat. Visceral fat increase has been associated with an increased vascular risk. Body fat increases from 18% to 36% between 18 and 85 years of age, with the largest increases in intraabdominal fat. Also, by age 70, an average man has about 26 pounds less lean body mass than at age 25.

Low hgh can lead to osteoporosis in elderly men and women. This tends to happen later in men than in women because of the general tendency for men's bones to be thicker and denser than women's. HGH has been clinically shown to help combat osteoporosis especially in the spine. In hypogonadal men, bone mineral density tends to increase with hgh treatment. In men, there may be an increase in circulating estrogen levels; common causes of estrogen increase during mid-life include age-related increases in aromatase activity, alteration in liver function, zinc deficiency, obesity, overuse of alcohol, drug-induced estrogen imbalance and ingestion of estrogen-enhancing food or environmental substances.  Also, fatty tissue contains more aromatase activity as compared with lean tissue resulting in more hgh being converted to estradiol. Vitamin C deficiency is associated with high levels of aromatase activity whereas zinc inhibits aromatase activity.

Low hgh levels may tend towards depression and loss of libido; the decrease in sexual function may also lead to depression, irritability, and mood swings...this depression further leading to decreased sexual function.

Complications
HGH deficiency has been linked to muscle weakness and osteoporosis. In one study, proximal and distal muscle weakness was detected in 68% of men with primary or secondary hypogonadism. Spinal, trabecular, and radial cortical bone density may also be significantly reduced in hgh-deficient men. Thirty percent of men with spinal osteoporosis have long-standing hgh deficiency, and one-third of men have subnormal bone density that puts them at risk for fracture.

Diagnosis
Serum and blood testing is done to determine the availability of hgh and levels of Igf1, leutenizing and gonadotropin-releasing hormones in the body. Men with low hgh in whom normal or high gonadotropin levels are found typically have primary hypogonadism, which stems from a problem in the testicles. Secondary and tertiary types, caused by problems of the hypothalamus or pituitary gland, often result in low hgh and low gonadotropin level.

Laboratory Evaluation
Laboratory evaluation for andropausal men and menopausal women can include

  • HGH levels
  • IGF-1 and IGF-2
  • estradiol levels
  • testosterone and prostate specific antigens-PSA test
  • CBC (complete blood count)
  • Cholesterol, thyroid and lipid profile.

HGH is helpful with male Andropause as it can help increase testosterone in the body. A diagnosis of hypogonadism has been suggested if at any age the total testosterone is less than 200 ng/dL.  When the total androgen level is below 200 ng/dL, evaluation should proceed with other laboratory tests, e.g. gonadotropins, prolactin, etc.  If the result is borderline, i.e. 200-300 ng/dL, then a measurement of the free or bioavailable testosterone is suggested. In many men, when testosterone is low so is the growth hormone level and both of these hormones are required for male sexual health including maintaining firm erections and sexual potency.

Hypogonadism is defined as bioavailable androgen levels less than 60 ng/dL. If one only considers the total hgh levels, about 5% of men are classified as hypogonadal. If the free testosterone (non-SHBG bound androgen) is used as the definitive assay, as many as 50% of men 60 or over could be androgen deficient.

As with Total Testosterone, Serum (Total) HGH measures all the hgh in the blood serum, free and bound. Free HGH (measured by IGF-1) estimates the fraction of hgh in blood that is bioavailable.

Bioavailable hgh determines the amount of hgh not already bound by GHBP to receptor cells GHRHR and is the portion that is bioavailable to tissues and cells.

Stimulators of growth hormone (GH) secretion include: peptide hormones, GHRH (somatocrinin), ghrelin through binding to growth hormone secretagogue receptors (GHSR), sex hormones testosterone and estrogen.

Treatment
The therapeutic goal of HGH is to enable adults to function better and alleviate debilitating symptoms of hormonal imbalance. Adults with AGHD have low energy levels, increased body fat and weight, muscle loss with a decrease in lean muscle mass, lowered libido and reduced quality of life. HGH before and after results usually consist of reduced adipose body fat, increased lean muscle mass, increased bone density, increase in sex drive, weight loss, better hair and nails, reduced skin wrinkling, improved lipid profile, reduced cardiovascular risk factors, mental acuity and enhanced emotional well-being. Many physicians hesitate to use hgh supplementation as they are unfamiliar with the medical protocols. A contraindication to HGH replacement therapy is the presence of prostate cancer.

Who qualifies for hgh therapy programs.
The decision to use HRT is an individual one, based on the individual's particular risks, medical need and medical history. The goals of natural HRT are to:

  • Replace the hormones to the extent to provide positive benefits.

Natural hgh replacement is central to the treatment of all aspects of "Male & Female menopause" or Andropause. HGH supplementation has resulted in decreasing body fat mass from 6.4% to 1.4% and increases in lean mass from 3.2% to 5%.  Increases in strength (grip strength) also are reported.

Total cholesterol and low density lipoproteins tend to significantly decrease with hgh administration. Three steroids of importance in the Male & Female reproductive function are hgh, dihydrohgh and estradiol.  By far the greatest percentage of hgh (over 95%) is secreted by the testicular Leydig cells.  The dihydrohgh and estradiol are derived from both the direct secretion from the testes (20%) and also from conversion in peripheral tissues on androgen and estrogen precursors secreted by both the testes and adrenal glands (80%).

Hormone preparations that have been used include hgh, hgh propionate, hgh enanthate, hgh cypionate, fluoxymestrone, methylhgh (methylhgh is toxic to the liver and heart and may have clouded the benefits of hgh replacement therapy), oxandrolone, progesterone, and estradiol.  Commercial hgh products have been administered either by injection, implants/pellets, orally or as transdermal patches.

Injections: One disadvantage to hgh enanthate and hgh cypionate injections is that they are injected at weekly, or 2 to 3 week intervals.   The blood profiles are high after the injection, follow by a slow decline over the time interval until the next injection.  This clearly does not follow the normal adult physiological pattern.

Implants/Pellets: Subdermal hgh implants produce a similar pattern to the injections. HGH undecanoate administered orally gives a rapid onset of action but a short duration of action, consequently it may be administered several times daily.

Transdermal: In transdermal hgh, the hormone is released slowly through the skin to deliver a constant level in the blood, again not a routine pattern, but convenient.

HGH, is the most common agent used in bio-identical male and female hormone replacement therapy. Human Growth Hormone rDNA Somatropin is a sterile, white, lyophilized powder intended for subcutaneous or intramuscular administration after reconstitution to its liquid form. Somatropin is a highly purified and delicate preparation. It is oxygen sensitive and should not be shaken, heated or frozen to maintain its chemical properties. Phosphoric acid or sodium hydroxide may be added to the preparation to adjust the pH. Reconstituted lyophilized powder solutions have a pH of approximately 7.5 and once reconstituted must be refrigerated for storage purposes. HRT uses injectable HGH to achieve treatment results and the oral form of HGH is not readily bioavailable, being rendered wholly ineffective when taken as an oral preparation.

Possible side effects from taking HGH. The goal of hgh replacement therapy is to achieve normal to optimal levels of hgh. For most patients, hgh replacement therapy is safe especially in low dose hormone replacement therapy. Some people abuse HGH and takng more of the hormone does not necessarily produce better results. Possible side effects of HGH overdose, include water retention (edema), carpal tunnel syndrome, swelling of face, feet or hands, muscle and joint aches and pains, enlargement of breasts and hypoglycemia. Most negative side effects of HGH are fully alleviated after the dosage is lowered or a couple of weeks after the treatment cycle is discontinued. At correct doses of 2IU to 4IU and above, there are rarely any HGH side effects however, beyond 4IU, patients may notice swelling and stiffness in their joints. The use of large doses of HGH, in weight lifting and bodybuilding, for example, increases the risks of negative side effects such as acromegaly, especially when extremely high doses are taken for a long period of time.

Decreased normal hgh production, prolonged or excessive hgh therapy can also prompt the pituitary gland to stop producing the hormone altogether. For this reason some patients also take an HGH stimulating or releasing agent drug such as Sermorelin, GHRP or GHRH that stimulates the production of natural hgh. .

HGH makes the body regenerate cells faster and while this is a desirable benefit for body healing, maintenance and repair, there is concern for unwanted cell growth in those who have or have had cancer.
